    Responsibilities

    NOTE: Preferred Experience: in management in clinical healthcare setting; non-institutional care programs including Home Based Primary Care with a Master of Science in Nursing (MSN) is preferred.
    Scope of Nurse Manager:
    The Geriatrics and Extended Care (GEC) and Home-Based Primary Care (HBPC) Service Line Manager executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care. The GEC and HBPC Service Line Manager reports to the Associate Director for Patient and Nursing Services.
    The responsibilities for the GEC/HBPC Nurse Manager include, but are not limited to the following:
    In collaboration with the Geriatrics and Extended Care Medical Chief and the Community Living Center Associate Chief Nurse, the incumbent manages the administrative and clinical aspects of the Geriatric and Extended Service Line including the Home Based Primary Care, Contract Nursing Home, Hospice & Palliative Care, Fee Skilled Home Care, Fee Homemaker Home Health Aide Program, Contract Adult Day Health Care, Medical Foster Home, Community Residential Care Program, State Veterans Home Program, Caregiver Support Program and the Geriatric Outpatient clinic.

    • The incumbent is responsible for coordinating interdisciplinary staffing, work assignments, budget, treatment services provided, and admission criteria of the programs.
    • The incumbent is responsible for the supervision of the various program managers (nurses and social workers) including the GEC Assistant Service Line Manager (social worker) and Caregiver Support Program Manager (social worker).
    • The incumbent works collaboratively with the Deputy Nurse Executive and Physician Service Chief to provide oversight for the Community Living Center.
    • The incumbent is responsible for various fund control points assigned to the program within the assigned areas.
    • The incumbent manages and controls the use of data related to these programs regarding day to day functioning and program evaluation. In conjunction with the Service Chief, plans, organizes and directs administrative, operational, patient and personnel activities for the service.
    • The incumbent provides leadership in the application of organizational processes and/or systems, improving outcomes at the program or service line level.
    • The incumbent is responsible to perform the following processes and their related activities: planning and designing care and services, directing care and services, integrating and coordinating care and services, and improving performance.
    • The incumbent assures that the Program is in constant state of readiness to meet Joint Commission Standards and VISN/local expectations for quality care.
    • The incumbent evaluates home care performance in relation to its mission, purpose, and objectives. The evaluation includes an analysis of the effectiveness of administrative practices, policies, and procedures including personnel, financial, and program administration practices.
    • The incumbent manages care across the health care continuum. He/she works closely with primary care provider(s) and other health care professionals and team members, other clinics, internal services and community agencies.
    • The incumbent is responsible for overall planning, coordination, implementation, management, and evaluation of clinical applications of GEC/HBPC within the organization, with particular emphasis on improving clinical quality and access to veterans.
    • The incumbent works closely with network and facility leaders and staff to incorporate new concepts and programs into extant systems, and provides education and training to all facility staff impacted by program, role, or technology changes.
    • The incumbent is knowledgeable of overall facility and network goals, missions and priorities to ensure that the GEC/HBPC programming does not impact adversely upon other programs or resources.
    • The incumbent is knowledgeable of funding sources and methodologies including Office of Rural Health, the area Rural Health Resource Center, Veterans Equitable Resource Allocation (VERA), third party collections, sharing agreement revenues, contracts and enhanced use revenue.
    • The incumbent has awareness of and participates in management discussions, decisions, and policy-making concerning GEC/HBPC and consults and advises the Quadrad.
    • The incumbent integrates program planning within facility operations, working collaboratively with the organization as a whole to ensure goals and priorities are complementary and in congruence with the overall VISN and VACO GEC/HBPC programs

    Qualifications Basic Requirements:
    • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
    • Education: Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
    • License: Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
    • English Language Proficiency: RNs appointed to direct patient care positions must be
      proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d).

    Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
    • Nurse I Level IAn Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
    • Nurse I Level IIAn ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
    • Nurse I Level IIIAn ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
    • Nurse IIA BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.

    Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment. Education
